Objective: To evaluate the effect of vaccine coverage of COVID-19 on the occurrence of deaths in elderly residents in the city of João Pessoa-PB. Methods: Quantitative ecological study, analyzing the correlation between deaths due to COVID-19 in the elderly (≥60 years) and vaccination coverage against the disease in residents of the municipality of João Pessoa-PB. All deaths aged 60 years or older reported for Severe Acute Respiratory Syndrome and positive for COVID-19 were included in the study with symptom onset data between February 15, 2020 and July 31, 2021, and all individuals of the same age group with a complete vaccination schedule for COVID-19 by July 31, 2021. Results: In the pre-vaccination period, 958 (55.0%) deaths were recorded in the elderly. And in the post-vaccination period, 783 (45.0%). In epidemiological week 32 of 2021, when vaccination coverage reached 89.3% of the fully vaccinated elderly population, 3 deaths were recorded from COVID-19. Simple linear regression showed that vaccination coverage for COVID-19 in the elderly impacts on the decrease in deaths in this population. Conclusion: The study identified the proportional decrease in deaths in the elderly when related to the advancement of vaccination coverage in this population.
