Abstract

Nursing care systematization (NCS) as a tool in the care of hospitalized adolescent is still underutilized by nurses in their routine. The objective was to apply an instrument of NCS, which comprises the steps of the nursing process to a teenager hospitalized with a diagnosis of sickle cell anemia. This instrument is based on basic human needs of Horta, which underlie the process of NCS used in the study hospital. NCS is composed of five phases: Historical nursing, nursing diagnoses, planning, intervention, and evolution. The diagnosis of a more severe general condition was pain, and care for the adolescent in pain relief has contributed to the achievement of results. The use of the instrument serves as a guide for the NCS to the hospitalized adolescent, allowing documentation of the actions clearly and objectively, facilitating communication and continuing care.
