THE USE OF SINGULAR THERAPEUTIC PROJECTS FOR CHRONIC NON-COMMUNICABLE DISEASES: AN EXPERIENCE REPORT

Abstract

Introduction: Singular Therapeutic Projects (STP) provide the possibility of changing the way how the health services are managed and organized, especially with regard to primary care, as they favor a change in the perspective of the professional on the individual’s complexity and on the importance of care. Thus, the STP is constituted as a particular tool to combat Chronic Non-Communicable Diseases (CNcD), considering the large number of patients and their associated comorbidities and functional limitations. Objective: This study aims to present an experience report based on the STP of patients with CNcD restricted to home, as part of the activities of the Education Program for Health Work (PET-SAÚDE). Experience Report: We observed difficulty in the establishment of bond with the caregivers, and these were found to be sickened due to the stress of caring. The comprehensive care of CNcD patients made us reflect about the prevention and treatment modalities, particularly for the population at social risk, as well as about the need to mobilize a health care network. The development of the STP allowed a constant and continuous relationship with the users, team and students; and after the strengthening of bonds, it made possible to modify their perspectives and implement changes. The perception of the value and the efficiency of building a care plan collectively contributed to a considerable involvement of the family heath unit team by taking into account the individuality of each person.
